Free resources
The educational worksheets can be read and printed without registering. They have blank spaces for completing on paper; the website does not collect the medication information you write on those printouts.
When you contact the Foundation
Following an email link opens your email application. If you send an inquiry, the Foundation receives the information you include, such as your name, email address, organization, and message. Use this channel for general inquiries, partnerships, volunteering, or donor questions.
Please do not email patient records, medication lists, passwords, or payment details. This website is not a clinical record system or a secure patient-message service.
Donation information
The giving link opens the Foundation’s existing Squarespace donation flow. Information entered there is handled through that checkout and its payment services. Review the checkout’s notices, amount, frequency, and any optional fee before confirming payment.
For donation records, receipt requests, or changes to recurring giving, contact the Foundation. Do not send full card or bank-account details by email.
Technical information and hosting
Website and payment-service providers may process technical information such as IP addresses, browser details, pages requested, and cookies needed to operate their services. Their own privacy notices explain their practices. See Squarespace’s privacy notice for the existing hosted website and donation platform.
External websites
Links to educational sources and Pocket Med List take you to separate websites with their own privacy practices. Chalice Health Foundation, Chalice Health LLC, and Pocket Med List LLC are separate entities.
Questions and requests
To ask what information the Foundation holds about you or request a correction or deletion, email info@chalicehealthfoundation.org. Describe your request without including sensitive personal information. The Foundation may need to verify a request and explain any recordkeeping limits that apply.
