First-year financial disclosure

Chalice Health Foundation was established in April 1, 2026. As of September 29, 2026, financial records and annual financial statements are not yet available for publication. This is a new organization in its first year; no historical income, expense, or program-spending figures are presented here.

Planned uses of support include developing health-education resources, community outreach, accessible program materials, coordination, and the organization’s operating needs. A percentage allocation has not been published. The Foundation does not claim that 100% of gifts go directly to program delivery.

Before making a restricted gift, please contact the Foundation to agree on its intended purpose. Requests for registration documents, leadership information, and financial-information updates can be sent to the Foundation.

How we will describe impact

Targets and plans are not completed results. This site does not claim a number of people served, medication errors prevented, hospitalizations avoided, or donations spent without documentation.

The proposed Medication Confidence program would track participation, lists prepared, follow-up participation, participant experience, and actual spending. Any future results should identify the reporting period and how they were measured.

Separate entities

Chalice Health Foundation, Chalice Health LLC, and Pocket Med List LLC are separate entities. Pocket Med List is operated by Pocket Med List LLC. Foundation donations should not be confused with app-subscription purchases.
