Established in April 1, 2026, Chalice Health Foundation is building its first year of community-focused work.

Turning a broad mission into a practical next step

Medication information can be difficult to keep organized, especially when a person sees several clinicians or has help from a caregiver. A clear list and prepared questions can support a more informed conversation.

Our next proposed initiative, Medication Confidence, focuses on free education and preparation. We are developing the program with the intention of involving licensed pharmacists, community hosts, and caregiver organizations.

Our approach

• Start with the person. Respect individual choices, priorities, and the people they choose to involve.
• Make room for different needs. Plan paper options, understandable language, and access without an app purchase.
• Keep the care team involved. Education supports conversations with clinicians; it does not replace diagnosis, prescribing, or medical care.
• Be clear about progress. Program plans are described as plans. Completed work should be documented before it is reported as impact.

Chalice Health Foundation, Chalice Health LLC, and Pocket Med List LLC are separate entities. Pocket Med List is operated by Pocket Med List LLC. The Foundation’s website is focused on its charitable mission, education, and community support.
