We are seeking community hosts, qualified volunteers, and funding partners. There are no announced sessions or open participant enrollment yet.
Who the program is designed for
Adults organizing multiple medicines, people preparing for healthcare visits, and caregivers who have permission to help. A smartphone would not be required.
The proposed experience
- Gather. Bring medication containers, over-the-counter products, supplements, existing instructions, and questions.
- Organize. Prepare a dated medication list. Mark information that needs a clinician’s confirmation instead of guessing.
- Understand. With a pharmacist, discuss existing instructions and use teach-back to confirm that the explanation was clear.
- Prepare. Choose questions for the next visit and record who to contact for clarification.
- Follow up. Plan a check-in to learn whether the list and preparation tools were useful.
Built for access
The proposed program would be free to participants. Paper materials would be available alongside optional digital tools. A donation, app account, or paid subscription would not be a condition of participation.
What we would measure
People participating; dated medication lists prepared; follow-up participation; caregiver involvement by consent; participant experience; and program spending. These are proposed measures, not results from an operating program.
Why this approach
AHRQ describes medication reviews that bring together medicines and supplements, and teach-back as a way to check whether information was explained clearly. These practices inform our proposed educational approach. They do not establish that this new program has achieved a clinical outcome.
