Pocket Med List is operated by Chalice Health Foundation, a 501(c)(3) nonprofit organization dedicated to supporting community health, medication education, digital health access, and healthcare navigation for underserved populations.

Pocket Med List is designed to help users organize medication information, set reminders, track medication routines, and share medication lists with invited caregivers or care team members.

Pocket Med List does not provide medical advice, diagnosis, treatment, prescribing, pharmacy services, or emergency medical services.

Pocket Med List is not directed to children under the age of 13.

Pocket Med List is a medication organization and reminder tool. It does not replace medical advice, diagnosis, treatment, medication instructions, or care from a licensed healthcare professional.

Always follow the instructions provided by your physician, pharmacist, or other licensed healthcare provider. Do not ignore or delay medical care because of information entered into or displayed by Pocket Med List.
