Delete Your Pocket Med List Account

You have the right to request deletion of your Pocket Med List account and associated data at any time.

Important Notice

Account deletion is permanent. Once your account deletion is completed, your personal information, medication records, reminder settings, care team connections, and app usage history associated with your account cannot be restored.

How to Delete Your Account

Option 1: Delete in the App

  1. Open Pocket Med List on your device.

  2. Tap Profile at the bottom of the screen.

  3. Select Account Settings.

  4. Tap Delete Account.

  5. Follow the prompts to confirm your request.

Option 2: Request Deletion by Email

Send your deletion request to:

support@chalicehealthfoundation.org

Use the subject line:

Delete My Account

Please send the request from the email address associated with your Pocket Med List account.

We will review and process verified deletion requests within a reasonable timeframe and, where applicable, within 30 days.

What Gets Deleted

When your account is deleted, we delete personal information associated with your Pocket Med List account, including:

  • Your name and email address

  • Medication names, dosages, schedules, and instructions you entered

  • Profile information you chose to add

  • Care team connections

  • Reminder settings

  • App usage history associated with your account

What May Be Retained

Some limited information may be retained if required by law, needed for security, fraud prevention, dispute resolution, or compliance purposes.

Aggregated or de-identified information that does not identify you may also be retained for analytics and app improvement.
